Job Title:        Registered Nurse
Reports To:    Director of Nursing/Nurse Manager

Summary:
The Registered Nurse is an integral part of the interdisciplinary care team and as such is responsible for coordinating the total care of the patient and family. Understanding that home care must be delivered according to accepted standards of practice, guided by the mission of the organization and the patient/family's needs and concerns, the Nurse blends all services into an integrated program of support.  The following list is not intended to be all inclusive and the Agency reserves the right to add, delete, amend, or modify the duties of this position.

Relationships:

  1. Supervises licensed practical nurses, and aides as assigned by the Director of Nursing.
  2. Liaisons with community referrals, agencies, health care professionals and families to provide greater understanding of home care.       


Qualifications:

  1. Current and valid Michigan registered Nursing license.
  2. Personal car for travel or other method of reliable and flexible transportation.
  3. Valid Michigan driver's license and current auto liability insurance.
  4. Training in infection control and universal precautions.


Primary Duties and Responsibilities:

  1. Provides admission information to the patient and family as required.
  2. Provides nursing assessments as directed by agency policy.
  3. Develops or assists in preparation of the client care plan.
  4. Provides patient care services as directed by the client care plan.
  5. Supervises licensed practical nurses on assigned patients.
  6. Supervises Aides on assigned patients.
  7. Teaches appropriate home care principles to family members, other health professionals and the community at large.
  8. Evaluates patient care services as directed by the client’s care plan and coordinate changes in the client’s care plan with the physician.
  9. Assures the evaluation at least every two weeks of Home Care Aides serving assigned patients.
  10. Reports changes in the patient's condition to the Director of Nursing or designee, and physician, as appropriate.
  11. Provides accurate, timely, appropriate documentation of all services provided.
  12. Attends meetings and inservices as required.
  13. Uses appropriate communication skills and provides emotional support to the patient/family as needed through good listening skills.
  14. Contributes to program development through building relationships with potential referral sources.
  15. Prepares clinical and progress notes.
  16. Counsels the patient and family in meeting nursing and related needs.
  17. Initiates appropriate preventative and rehabilitative nursing procedures.
  18. Follows all appropriate policies and procedures.
  19. All other duties as assigned.


Knowledge and Abilities:

  1. Ability to develop good rapport with patients/families, team members and other health care professionals.
  2. Ability to assess needs of the patient/family quickly and accurately.
  3. Sound knowledge of home health care.
  4. Knowledge of the nursing process and accepted standards of nursing practice.
  5. Ability to exercise good judgment.
  6. Excellent communications skills.
  7. Ability to respond appropriately in crisis situations.
Measurement:
  1. Quality Patient Care
  2. Nursing assessments in 100% of charts reviewed as monitored by the initial chart audit, quarterly chart, audit and random audits.
  3. Performs visits in accordance with ordered frequencies as evidenced by 100% of charts reviewed.
  4. Timely, accurate, and informative documentation.
  5. Lack of chart deficiencies reported to the Administrator and/or Coordinator.
  6. Home Health Aide supervisory visits.
  7. Timely evaluation of the plan of care. (At least every two weeks)
  8. Positive comments on service or lack of complaints from families either directly or through the Service Satisfaction Questionnaire.
