Primary Position Duties:

  • Review all OASIS Assessments
  • Generate Plan of Care
  • Ensure that Home Health Clinician’s Documentation meets Medicare Conditions of Participation and Rules and Regulations
  • Provides total oversight of Patient Care through documentation review – includes Home Health Clinician’s Observation and Assessment of patient’s disease condition, Management and Evaluation of the Plan of Care, Patient Teaching and compliance to Teachings
  • Ensures Care Coordination with the Physician regarding patient’s progress (or lack of), patient’s response to current doses of prescribed medications, and Medication Management and Reconciliation. 

Education or Skills:

  • High School Diploma or GED
  • 3 years relevant experience
  • Fluency in English (Speaking – Reading - Writing)
  • Able to follow policies and procedures and provide detailed documentation
  • Possess the ability to work independently
  • Exceptional understanding and working knowledge of Medicare Conditions of Participation, Rules, and Regulations required
  • Ability to make appropriate clinical judgments
  • Good written and oral communication skills; interpersonal communication and teaching skills
  • Ability to manage a variety of assignments while establishing priorities and assuring that necessary duties are completed

Position Requirements:

  • Home Health or Hospice experience
  • Synergy EMR experience
  • SOC RN, LVN/LPN, PT, OT, SLP licensure in any of the 50 US states
  • Certified Case Manager
  • Medical or Coding certification

Position Summary:

HealthCare Synergy, Inc. Clinical Case Managers oversee the implementation, evaluation and supervision of the care provided to clients in their own homes.
